One patient came to Go Physical Therapy struggling with chronic neck pain and tension headaches, caused by muscular imbalances in the upper back and neck — a condition often referred to as Upper Cross Syndrome. This condition typically presents with tightness in the upper trapezius, levator scapula, pectoralis major and minor, and suboccipital muscles, while key stabilizing muscles like the deep cervical flexors, middle and lower trapezius, rhomboids, and serratus anterior remain weak. The Treatment Process

  1. The patient’s treatment began with heat therapy to relax the neck and shoulder muscles, followed by cupping to increase blood flow and reduce muscle tension. Myofascial release was applied to the upper trapezius, levator scapula, pectoral muscles, and suboccipital muscles to release fascial restrictions and lengthen shortened tissues, which helped alleviate headache symptoms.

  6. Resistance band “TY” raises to strengthen middle and lower trapezius

  7. Scapular retractions and wall slides to engage rhomboids and serratus anterior

  8. Neuromuscular re-education was incorporated to retrain posture, coordination, and proprioception. This included cervical retraction drills, wall posture alignment exercises, and diaphragmatic breathing to reduce overuse of accessory neck muscles.

  9. Finally, the therapy emphasized functional integration and ergonomic correction, reinforcing neutral head and shoulder alignment, optimizing workstation setup, and promoting posture awareness throughout daily activities.
